Moving to Metric-Only Measurements In Patient Care

SETON FAMILY OF HOSPITALS BEST PRACTICE:  METRIC-ONLY MEASUREMENT FOR PATIENT CARE 

Medication errors have resulted in serious harm and even death when pounds and metric measurement have been confused or inaccurately charted. 

To support our continuous drive to ZERO preventable harm to our patients, Seton Healthcare Family has been in the process of converting to the use of only metric measurements. 

  • All in-patient scales locked to only measure in metric measurement
    • Goal:  on-track to 90% conversion by October 31, 2015 
  • Electronic medical record to be changed to only accept weights in metric measurement 
  • All physician forms will now specify weight in metric measurement 

Seton is undertaking this initiative in collaboration with all Ascension Health Ministries, working from the evidence-based recommendations of the Institute for Safe Medication Practices (ISMP).
